Vermont requires:
- CES Report Application from CGFNS
- Vermont NCLEX Application Form
- Passport in lieu of SSN
- Payment thru money order, bankdraft.
Additional Notes:
- Vermont NCLEX Renewal is every odd year (e.g. 2011, 2013…)
- You can put your Vermont license in Inactive status.
