Starting 01 October 2008, all incoming nurse-applicants of Vermont Board of Nursing will submit the revised Application Form posted in their website, www.vtprofessionals.org.

As of October 1st, all incoming applications must be the current application form and verification of education form.  All new requirements apply to applications received October 1st and after.

Aside from the Verification of Education (2 pages), the new form added the Verification of Licensure form to be submitted by the applicant to PRC, then the PRC will fill up the form and certify your license (or if you dont have the license yet, the board exam passing letter). You can now include these forms with your application in mailing, provided these are in separate sealed envelope from PRC. Same with your Verification of Education forms, TOR , RLE summary and detailed Course descriptions. In short, you can now have one mailing packet for all documents and payment, as long as the PRC forms and docs are in one sealed envelope, and the School forms and docs are in another one sealed envelope.

As much as Im giving you tips here, it would do you well if you read the detailed instructions and checklist in the downloadable Vermont Application form packet, here. Again these changes are applicable to all incoming applications starting October 1, 2008.
