Firstly, obtain a copy of Application Form from the CA BON Website. It’s 26 pages and in pdf format, so you need Adobe Reader to open it. Pages 1-10 are General Information and Instructions. You have to fill up starting page 11, as follows:
APPLICATION FOR LICENSURE BY EXAMINATION (pp. 11-12)
Basically, this is all about your personal details. Fill in the necessary information and here are some tips and frequently asked questions I encountered when filling up this form.
- Social Security Number: Leave it blank, unless you have legally work in the US and have secure one.
- Place your 2×2 picture on the second page beside the date on the lower part of the page using tape NOT glue.
- Interim Permit: This is an option to obtain temporary permit while waiting for the results of your exam, valid for 6 months or until the release of results. Costs extra $35 so we dont normally avail this.
The rest of the items are self explanatory.
REQUEST FOR TRANSCRIPT (p. 13)
You have fill out the personal information and submit this form to your school registrar and have them fill up the rest. The school will be the one to submit this to Ca BON together with your Transcript of Records and RLE Summary.
BREAKDOWN OF EDUCATIONAL PROGRAM FOR INTERNATIONAL NURSING PROGRAMS and NURSING PROGRAM VERIFICATION (pp 17,22)
Fill out the personal information(upper part) and have the school fill out the rest. Usually they mail it together with your TOR which was filled by the school registrar.
COPY OF LOCAL LICENSE OR LETTER OF EXPLANATION
If you have your license already then just scan it and print it or photocopy it back to back and attach it together with the other documents you will submit. If you do not have your local license yet just request PRC of your Certificate of Board Passing and Board Rating and write a simple letter of explanation.
FINGERPRINT CARD
You would have to request this separately. You can request by going to this link: https://app.dca.ca.gov/rn/requests.asp
Fill out the online request form and then they will mail it to the address you provided in 2-3 weeks. Then you can go to NBI offices and request an agent to supervise your fingerprinting.
Place your accomplished fingerprint card together with other forms and payment. Do not bend or fold the fingerprint card, or else they will not accept it and mail it back to you. I suggest you find any hard envelope for your documents.
PAYMENT
Submit the correct TOTAL FEE with your application, made payable to the Board of Registered Nursing by check or money order (bank draft) in US Currency. Total fee for Application and Fingerprint card processing is $126.
So to summarize:
Packet A. Documents that YOU will submit to California BON:
· Application for Licensure by Examination (2 pages)
· Letter of Explanation/ Copy of PRC License
· Fingerprint Card (2)
Packet B. Documents that the SCHOOL will mail to California BON:
· Request for Transcript form + TOR and RLE Records
· Breakdown of Educational Program for International Nursing Programs Form
· Nursing Program Verification Form
Tip: Since most of the schools don’t mail the above documents but instead they give it back to you with your school envelope, then you have to make it appear that the documents were mailed by the school. Meaning, you have to mail those documents in Packet B SEPARATELY from the documents in Packet A. Also, you have to mail those documents (B) in the same city/town where your school is located.
Processing time of your California eligibility ranges from 4-6 months.
As much as I’m giving you hints on the California Licensure by exam processing, it would be a great help if you read the instructions on the application form packet. If you have further questions don’t hesitate to drop a comment.

One fingerprint card would suffice. Just make sure it is not folded, crumpled. Also, fingerprints and entries must be in black ink.

Based on their instructions, the school must directly send those docs. But since most of our schools here dont send them but instead return those docs to nurses, you must make it appear as though the school mailed them by sending it separately, and from the city where your school is located.

Step 1, you should contact Cali BRN to rectify any error in your email add. Then go to this link of pearsonvue.com: http://pearsonvue.com/contact/asiapac/customerservice/ Fill in your correct details (esp. the correct email add) and on the comment box tell them that you have registered for NCLEX-RN Testing and paid $200 but havent received your Registration Confirmation and ATT due to incorrect email. Then kindly request them to send those emails to your correct email address(specify). Wait 1-2 days, they usually send it.

Ipapadala mo both, but separate mailing. Then they will review your papers and make you eligible to test. Then thats the time you register with pearsonvue for nclex testing.

You have to go through the application process before taking the NCLEX. If you want to apply at CA Board of Nursing, you can explain to them that you dont intend to practice nursing in the Phils thats why you didnt take the NLE. Please read the instructions above on how to go thru with the process.

i already passed the nclex ca board since 2005 and still waiting for my visa apointment and i’ve already requested for an extension of my nclex pass to be considered until sept09. Kung d pa rin ako makapasok and makaget ng ssn by then, pwede p kaya ako magrequest uli ng another extension ng pass mark ko? with the current trend of visa processing and retrogression mukhang malabo pa akong matawag for interview. what do you suggest i do?

Accdg to CA BRN Instruction, ‘transcripts are required from all colleges and/or universities you attended that reflect courses required for a degree in nursing, including general education course requirements and all nursing courses.’ So we have to follow that.

It must appear that the docs came from the school, so pagmaliit yung envelope pagkasyahin mo na lang. Anyway if you mail it by courier, they usually enclose it in another pack.

Kailangan po bang kasama ang RLE sa ipapadala sa BON kasi po hindi naman ito nakalagay sa applicastion form na nadownload ko sa website nila ang sabi lang po ay Breakdown of Educational Program for International Nursing Programs at TOR kasama ng Request for Transcript wala pong sinabi na RLE. Yes, you must send the RLE summary. It’s indicated in the instructions that you must send transcripts and all coursework. Again, let the school send those or mail it separately if you’re the one to send it.

Eligibility in California is used to be at 2 years. But Ive heard reports recently that it was shortened to a year. But the ATT is valid for a year. Just check your eligibility letter.

hi.. i’m kind of confused kc po sa website ng california rn, $200 ung bbyaran sa application for exam, and here $126, how much po b tlga ung dpt ilagay na amount sa cheque, ung exact amount po for appli. and finger scan card po? n also, complete na din po ung mga reqs ko, however ung license and certification of passing ndi pa po narrelease ng prc; pwd po b mgpa NOTARIZE nlng indicating that i really passed, pra mamail ko na ung nclex application ko 4 CA..? In the first page of the Application Forms and Instructions, its $126 for Application and Fingerprinting. Yung $200 later na, thats for NCLEX Registration sa Pearsonvue. If you dont have your license yet, you have to send a letter of explanation stating that you already passed the NLE. You can attach your certificate of passing/board rating as a proof, and PRC issues those certificates sooner than the license.

is it better to apply for nclex here in the philippines or in the US?
coz im planning on going there this august, i just want to know if theres a difference if i apply here or there? would the process be faster? would it be cheaper? just weighing the pros and cons..
and about the ssn, should i put my ssn that my parents got for me when i was a child while living in the states in the ’80s? but now i only have a tourist visa.. would there be a difference?
is the reason “i do not plan to practice nursing in the philippines” enough to convince the board for me to take the nclex?

Well, mailing your application would be faster if you’re there already. But you have to process school docs and forms here in the phils.
Is your SSN still valid? If it is then you can use that.
It depends on the CA BRN if they will accept that reason, but we had applicants before who used the same reason and was approved.

Walang form na galing California para sa PRC. If you dont have a license, just have an explanation letter and the certificate of passing/board rating from prc to prove that you have passed the nle and your license is on process.

hi nice thing i saw this site because i myself also have a problem i recieved an initial evaluation of my application for examination mark by x that ,tor both theory and clinical hours should be sent directly to the board which i did along with the clinical portion of transcript or rle , educatioanl breakdown form, but all of those were mark by an x,my friend and i have the same big envelope with different sobre hers never had any problem we have the same complete requiremnts but mine does,how did it happen? wahat will i do ,what are the steps i need to follow ? please help !.. thanks god bless..

In the retake form, use your married name but be sure to fill up the ‘Previous Name’ field with your full maiden name. You can also attach this form from CA BRN. As a proof attach a copy of marriage certificate with the forms and payment. It would be easier for CA BRN if you attach a simple letter explaining your status.

Yes, you can now register with pearsonvue for NCLEX-RN testing para ma issue na nila yung ATT mo, which will be valid for a year.

Actually, its not about nclex expiration. It’s about the SSN requirement. You’re given 3 yrs to comply with that after passing the nclex, or else your files will be destroyed.

According to the Ca instructions, if you dont have a license you must attach an explanation letter why is that so. It’s up to Ca BRN if they will accept your reason. There were cases before that they told Ca BRN that they dont intend to practice nursing in the Phils thats why they didnt take the NLE, and the Ca considered them and given approval.

hi.. regarding po dun sa nclex application i’d like to verify if tama ung intindi ko sa mga explanation mo here at ur forum.. 1 person hir said that after a month of applying 4 nclex, she got the eligibility.. e2 po ba ung basis for me to take the exam, meaning the “go signal” na pwd na mgtake ryt away after paying/registering to pearsonvue my chosen exam d8? or i will have to wait for few wks or months pa uli for them to send back another mail to me w/c will indic8 that my chosen d8 to take the exam has been approved? so all in all po ba from d tym u submit d application up2 the approval to take from d chosen date, will take how long? coz im planning 2 review na sna while w8ng. n i hope u can give me suggestions on wer 2 review 4 nclex, ung reliable po? If you have received your eligibility letter from California, you can now register and pay at pearsonvue. Then they will issue your ATT in few hours. Pag may ATT ka na, you can schedule and take your NCLEX anytime within the 1-year validity. So yung hinihintay lang natin e yung eligibility letter; you can be approved you and made eligible as early as a month or as late as 8 months after sending your application.

Here’s the retake form. Fill this up and send to California plus the $75 retake fee, 45 days after receiving your nclex result. Then in about a month, California will issue you another eligibility letter.

No, based on the first page of CA application instructions, you only have to pay the application fee of $75. Yung $126 is $75 for application fee and $51 for fingerprint card.

i have relatives living in california, can i ask them to pay the application fee instead of sending it through bank or cheque??
and share ko lang, i received my fingerprint card a while ago (i requested it about a month ago) but to my dismay the postman folded the envelope, you can really tell how the postman roughly handled the package through the appearance of the envelope.. the cards are badly crumpled, i ended up requesting for another copy again.. T.T

sir nonoy,tanong ko lang po kung ung fingerprint card ilalagay ko sa 9×12 envelop,so bale nsa loob po sya ng envelop/packet A?anong enevelop ang dapat gamitin pra sa packet A?pwde bang mas maliit sa 9×12 ung envelop ng FP card pra magkasya sa envelop/packet A?

Wag mo na ilagay sa separate envelope ang FP card, isama mo na sa envelope ng packet A. Look for hard envelope so that the FP card will not be folded.

Good day po! First of all, I’d like to thank you for this post! It was really helpful when I was still applying for NCLEX. Now that I have my ETT, Im a little confused. I also applied for an interim permit kase. What in the world is this good for? It’s not yet activated. It says I need to go to the US for this to be activated? So now I need to take the NCLEX outside the Phils? I read a rumor that it is easier to pass the nclex in the us than in the phils. Is it true? Im a US citizen and yes, I already have a SSN. Thanks so much!

Interim permit is for those who want to work there temporarily before being licensed. I think its valid for six months. Regarding the testing center, its your choice where you want to take the NCLEX. If its easier in the US, I dont know.

ask ko lng po sir, yung fingerprint ba na ipapadala nila is thru fedex or snail mail? kc ang nilgay kung adres nung nag request ako ay yung adress ng province namin, at wlang fedex dun sa amin..
or pwede ba kaya mag request uli ako then address na sa city yung ilalagay ko?

its me again sir.tanong ko po sana sir kung what po ilalagay ko na date sa 2nd choice doon sa qualification examination locations and dates kasi ang first choice ko po eh sa nov. 11 pero po yong 2nd choice pang next year na po ata yon.will I leave it blank sir or lagyan ko na lang po ng march 2010 doon sir without the date. At ano po yong exam center number?

Yes, you can put March 2010. You can find the exam center number in page 19 of application form or in the CGFNS website.
Regarding the name of the diploma, ilagay mo kung ano ang nakasulat sa diploma mo. Usually, kung primary/elementary and HS/secondary we just put ‘Diploma’, but in nursing certificate its ‘Bachelor of Science in Nursing’. Tapos you can check the degree obtained space across it.

hi again…i received a letter from cali BON regarding my application for nclex exam…they said that they did not receive a copy of the requirements from the school registrar..but when i went to our school registrar they presented to me the return card na may date kung kelan na recieve ng cali BON ung mga docs na pinadala…what should i do? should i request again from my school to send another copy o wait lang ako ulit ng next letter?

hello again sir..I have again a question po regarding po sa application form…doon po ba sa nursing education sir, will I mark all the box po ba? tapos po ano po ang ilalagay ko sa nursing experience po kung di pa nman po ako nagkaroon ng experience except noong student pa lang po ako. And sa mga quetion po sa nursing registration or license po ano po ang mga isasagot ko po.. pasensiya na po kayo sir ta madami po ako tanong. Ayaw ko lang po kasing magkamali sa pagfill-up po baka po kasi magkaproblem.Wala na po kasi akong ibang pwedeng pagtanungan regarding this po. kayo lang po talaga sir. kaya po Im very thankfulpo sa inyo and sa site nyo pong ito. SANA MATULUNGAN MO RIN ME.. ask lg sana kasi mjo in doubt pa rin me now…sa texas ko talaga gsto magwrk gawa ng marami me relatives dun at isa pa kasapi xa sa Compact States..pro ang prob if directly to texas mahal kc may CES pa..kung endorsement wala na yun diba?ok lg if tru california me muna kahit na maging prob ko later on ung SSN naman? na kelangan sa endorsement to texas..papano ko makkuha kaya un? pwd kaya pag may direct employer me sa texas, makakakuha din me SSN na pd ko makuha license sa cali? gosh help..GODBLESS U PO AND MORE POWER!

Yes, no CES for endorsements. I suggest you apply initially at states that issue license without SSN. Yung california kasi walang license without SSN e.

isa pa kung sana matulungan mo rin me maraming mraming salamat…gaya ng dinadaing ng iba, not clear pa sa akin..kasi nag umpisa me mag nursing sa La Salle d2 provnce, 2000-2003 natapos ko ung associate degree or ASHE..tas nagtransfer me St John and Paul Colleges DUN SA LAGUNA pro 1wk lg stay ko dun,winidraw ko ung subject pro drop ung niligay talaga nila(kainis NA SKUL BUTI PINASARA NA SILA) SORRY FOR THAT….tas ngtransfer me uli blik sa hometwn ko to another skul nga lg frm 2005-2008 Riverside College..nacredit naman lahat ng subjects ko pati drpd subjcts nkalagay sa T.O.R ko huhuhu..okay lg enwei un nga KELANGAN KO PA BA MAGREQUEST NG T.O.R SA BAWAT SKUL OR HINDI NA ????

good day sir…ask ko lang po sir kung yong mga personal files ko po ba ay pwedeng yong tita ko po sa state magforward sa cgfns mailing address. tapos po yong galing sa school at prc po ba pwedeng siya na din magpada doon? or kailangan po na manggaling dito sa phil. yong sa school at prc? at pwede po bang via airmail lang yong pagpadalhan noon at hindi via courier kasi po masyadong mahal pag sa fed ex po. tnx sir
